How to Obtain a Copy of Your Medical Record

In order to protect your privacy, we require written authorization and a valid photo ID to release your medical records. Below is the Authorization for Release of Medical Information. Please print and complete the form. Bring the completed form, along with a valid photo ID to the Medical Records Department at 645 S. Central Avenue, Chicago, Illinois 60644.  You may also obtain a copy by contacting the Medical Records Department at (773) 854-5376 or (773) 854-5370.

Office Hours

The Medical Records Department is available to assist you during regular business hours, Monday through Friday from 8:00 a.m. to 4:30 p.m.

When Will I Receive a Copy of the Medical Record?

Copies are processed within 7 to10 business days from the date the request is received, but can take up to 30 days.

Is there a Fee Associated with Obtaining a Copy of My Medical Record?
