The medical records department plays a key role in caring for our patients because it tracks every treatment, procedure and diagnosis by physicians for each patient. How to Obtain a Copy of Your Medical Record

In order to protect your privacy, we require written authorization to release your medical records. Below is the Authorization for Release of Medical Information. Please print and complete the form, and return it to the Medical Records Department as requested.

Acceptable Forms of Identification

The following are acceptable forms of identification:

  1. State-issued ID
  2. Driver’s License
  3. Passport

  2. Social Security Card

Office Hours

The Medical Records Department is available to assist you during regular business hours, Monday through Friday from 8:00 a.m. to 4:30 p.m.

When Will I Receive a Copy of the Medical Record?

Copies are processed within 7 to10 business days from the date the request is received, but can take up to 30 days.
